Enrollment Information
Please view the documentation below to enroll your child in a MUSD school in preparation for enrollment. Remember, enrollment cannot be finalized until ALL required documentation is presented at your school of enrollment.
When registering your child, who has not previously been enrolled, the following documents are required:
1. Photo ID
- Driver's License
- State ID issued from DMV
- Passport
- Government ID
- U.S. Military ID
- Government Employee ID (city, county, state, or federal)
2. Proof of Age
An original birth certificate is the primary source of evidence of age. Other documents, such as a passport, baptism, and hospital certificate, are also acceptable.

3. Immunization Requirements

4. Proof of Residency
Two (2) original, current utility bills or one (1) original, current utility bill plus Escrow papers, or one (1) current utility bill plus a Rental Agreement in the parent's/legal guardian's name.
5. Dental Examination
State Law requires a dental examination for Transitional Kindergarten and Kindergarten on first school entry. This assessment may be done within 12 months before kindergarten admission but not later than May 31 following school admission.
