Annual Transportation Registration Form

 

All eligible* students MUST REGISTER each year to receive bus service for the 2023-24 school year.  Students will not be scheduled on bus routes unless we receive this completed form.  This registration process will allow us to plan bus routes more efficiently.  If your student will require transportation to and from school for the coming year, please complete a form for EACH STUDENT and return it to your school office or MUSD Transportation Dept., 325 E. Huntington Dr., Monrovia, 91016. 

If you do not register for transportation at this time, you may re-establish bus service by contacting the Transportation Department at 626-471-2913 at any time during the school year.  Your student will be issued a Transportation Identification Card on the first day of school to ride the bus to and from school.  The school site will replace this card one time at no change.  Replacement cards after that will cost $3.00 each.

Note: If you move, you must contact your school to fill out a new Transportation Registration Form.  You will be re-routed and contacted as soon as the Transportation Department receives the form from your school with your new address.
